Ambition 24Locums provides GP`s to general practices across the UK.
In order to register with us, we would like you to follow the procedures below. Once you have all the information required you can register with any of our branches across the UK.

Prior to registration please obtain the following information:
  1.  Instigate an Enhanced Disclosure via the Criminal Records Bureau, tel: 0870 909 0811 quoting Ambition Recruitment Services, reference no.       20165500005. Complete the form when it arrives in black ink and bring it to your interview together with a cheque for £36.00 payable to the       CRB.
  2.  Download a Checklist which details all documents and information that we will need to see.
  3.  Download and complete fully our Application Form.

  4.  Download our Contract for Services.
  5.  Obtain proof of your professional indemnity
  6.  Download and complete the Professional Indemnity Disclaimer.
  7.  Obtain your PMETB / JCPTGP Certificate of Aquired Rights
  8.  Details of the Supplementary List / PCT Inclusion List Letter.
  9.  Foreign Nationals - Police Check requirements.
